• Photo ID
• Insurance card
• List of current medications (including dosages)
• Referral paperwork (if your insurance requires one)
• Any recent X-rays or imaging (on disc or digital file is fine)
• A list of questions you want to ask Dr. Catlett

If you’ve had previous hip or knee surgery anywhere, bringing those records helps Dr. Catlett give you the most accurate assessment at your first visit.
